Wesley Diabetes Service

The Wesley Diabetes Outpatient Service is comprised of Diabetes Educators who focus on helping people with diabetes self manage their diabetes effectively and prevent complications. Diabetes Educators provide information, support and motivation to patients and carers to ensure patients feel empowered to self-manage their diabetes.

Working in conjunction with medical specialist and other healthcare workers our Diabetes Educators play an important role in The Wesley hospital multidisciplinary approach to healthcare to achieve optimal patient outcomes

Our service
A Credentialed Diabetes Educator is available to provide consultations for the following:
- Preconception care for women with pre-existing diabetes
- Gestational Diabetes Mellitus
- Type 2 Diabetes
- Type 1 Diabetes
- National Diabetes Service Scheme (NDSS) registration
- Pump Start Ups
- Education and assistance with diabetes self-management skills
– help with blood glucose monitoring
– insulin initiation
– new technology
– CGM
