Hospital in the Home provides you with the professional and personalised healthcare for which The Wesley Hospital is renowned in the comfort and privacy of your own home, allowing you to get back to doing what matters to you most. Our Hospital in the Home service is delivered in partnership with Blue Care, a trusted community and home care provider that is also part of the UnitingCare family.
Eligible patients will receive a personalised care plan developed by our highly qualified doctors, nurses and allied health staff here at The Wesley. Your care plan will then be administered by the friendly and professional Blue Care nurses, who will treat you in your home.
Your Hospital in the Home clinical manager determines your eligibility to receive treatment safely at home. The Hospital in the Home team liaises with The Wesley Hospital team of doctors, nurses and allied health professionals to determine your personalised care plan. Arrangements are then made for you to transfer home and continue your treatment.
Your treating team will maintain daily communication with you while you are receiving Hospital in the Home. Your personalised care plan is delivered by the Hospital in the Home team, with your progress monitored carefully until a full recovery is made.
Hospital in the Home greatly decreases the amount of disruption your healthcare treatment has on your life. With the Hospital in the Home service, you receive The Wesley’s quality care within the comfort and privacy of your own home, meaning you can get back to doing what matters to you most.
This forms part of your assessment if Hospital in the Home is an option for you. Provided you are able to communicate with the Hospital in the Home team and seek assistance if required, there is generally no requirement for another person to remain at home with you.
This depends on your healthcare needs. You will receive a minimum of a daily visit from our Hospital in the Home nurses. In addition, 24-hour on-call support is available, and you will be provided with information about how to contact the service.
Access to Hospital in the Home (HITH) is healthcare needs based and depends upon availability of funding from your health fund. HITH may be suitable for a range of health conditions, including respiratory conditions, skin conditions, other infections or complex wound management.

We will work with your health fund to determine your eligibility and approval. In most instances, the cost is the same as receiving in-hospital treatment and is covered by your private health fund.
